When hepatitis becomes long-term, especially in hepatitis B or C, the liver gets inflamed again and again. Over time, this can cause scarring of the liver (called fibrosis or cirrhosis), which weakens how well it works.

Hormonal Imbalance

A large study [1] published in The Journal of Sexual Medicine looked at 673 patients with chronic hepatitis B (CHB). The results showed that 25.4% of patients had sexual dysfunction, and the problem became more common as liver health declined.

People with greater liver fibrosis and depression were also more likely to face sexual problems. The research found that liver damage, hormonal changes, and depression were the main causes of sexual dysfunction in these patients.

Another study [2] also found that men with hepatitis B or C were about 1.5 times more likely to have erectile dysfunction than men without hepatitis.

Blood Flow Problems

Sexual function depends on vascular (blood vessel) health. Chronic hepatitis leads to systemic inflammation that reduces NO production and damages blood vessel walls, known as endothelial dysfunction.

Research shows [2] that when a person has hepatitis, the liver gets inflamed. This inflammation causes stress in the body and reduces the liver’s ability to make important proteins. Psychological Factors

Living with chronic hepatitis can also affect a person psychologically. Constant fatigue, social withdrawal, and fear of disease spread can often cause stress and depression, which further worsen sexual performance.

In a 2022 study [3], the rate of sexual dysfunction increased from 18.1% in patients without depression to 34.2% in those with moderate-to-severe depression. This means that even if hormonal or vascular functions are normal, the mental stress of hepatitis can still lead to ED.

Another study [4] found similar results for chronic hepatitis, where up to 40% of men experienced erectile problems, with depression and chronic fatigue being major co-factors.

Older Men and Chronic Hepatitis

A 2023 study [5] done on men over 65 years showed that those with viral hepatitis had significantly higher rates of ED than healthy men of the same age, even after excluding diabetes and obesity.

This suggests that hepatitis itself is independent of other diseases and directly affects erectile function in older adults through hormonal and vascular mechanisms.

Can Erectile Dysfunction Improve After Hepatitis Treatment?

Yes, in many cases, ED can improve once the hepatitis infection is successfully treated and the liver starts to recover. One study [6] on men with hepatitis C treated with direct-acting antivirals (DAAs) found that testosterone levels and sexual satisfaction improved significantly after treating hepatitis. However, this improvement depends on how much damage has occurred to the liver. In severe stages, hormone recovery may be incomplete. But, managing the infection early greatly increases the chances of regaining normal sexual function.

How can hepatitis caused ED be treated?

Doctors suggest following a comprehensive treatment plan rather than focusing only on sexual symptoms.  The key steps for this include:

  1. Treating the underlying hepatitis using antiviral therapy (for hepatitis B or C) to stop disease progression.
  2. Balancing hormones for checking testosterone levels and treating hypogonadism if necessary.
  3. Addressing psychological health with the help of therapy or counseling for depression and anxiety.
  4. Lifestyle changes like avoiding alcohol and smoking, eating a protein-rich diet, and maintaining a healthy weight to support liver function.
  5. Medical management of ED can be done when needed, phosphodiesterase inhibitors (like sildenafil) can be prescribed after liver safety evaluation.
